HOW WE TREAT SLEEP APNEA

A good night's sleep is possible. We want to get you there.

Whether you know or suspect you have sleep apnea, we'll help you pinpoint the severity of your case. We'll start by assessing your likelihood based on risk factors such as weight, snoring, daytime somnolence, fatigue, and mental fog. Then we'll send a sleep study device to your home. You'll wear the device for one night in the comfort of your own bed, send it back to us, and a pulmonologist will review your results. Once we have the data, we'll book a follow up virtual visit to discuss your treatment options.

DR. MIKE PHELPS, MD

Dr. Phelps is a Portland native, growing up a few minutes from the Bridgeport Family Medicine office.  He attended medical school at OHSU in Portland and completed his family medicine residency training at Peacehealth Southwest in Vancouver, Washington, where he served as Chief Resident.  He lives with his wife and 2 children in SE Portland.

DR. STAN ADAMEK, DO

Dr. Adamek was raised free range in Southwestern Oregon. Attended Linfield College where he received a degree in exercise science. He then went to medical school at A.T. Still University in Arizona where he was trained in osteopathic manual medicine as well as traditional western medicine.  He completed his family medicine training at Peace Health SW in Vancouver, Washington. He lives in Wilsonville with his 2 children.

DR. ANDREA TREMAINE, MD

Dr. Tremaine was born and raised in the Alaskan Arctic. She moved to Oregon in high school and graduated from Newberg High and then from George Fox University with a degree in Biochemistry. She attended medical school at Loma Linda University located in Southern California before deciding she was ready to move back to the Pacific Northwest to complete her family medicine residency training at PeaceHealth in Vancouver, WA.

DR. LORNA SHANKS, MD

Dr. Lorna Shanks made the Pacific Northwest her home when she enrolled in the University of Puget Sound to pursue an undergraduate degree in Spanish. She attended medical school at the University of Washington and completed her family medicine residency training at PeaceHealth Southwest in Vancouver. Dr. Shanks focuses on helping patients make individual choices in line with their goals and guides them through the treatment process when chronic illnesses develop.

HANNAH JONES, PA-C

Hannah is a native Oregonian, born and raised in Salem. She graduated from Oregon State University with degrees in Biochemistry/Biophysics, International Studies, and Spanish. After working in research at Tufts University in Boston and OHSU in Portland, Hannah received her Master’s in Physician Assistant Studies at Idaho State University.  Now back in the Pacific Northwest, Hannah enjoys playing volleyball, traveling, and hiking with her husband and son.

LINDSAY KLEPS, NP

Lindsay is a Family Nurse Practitioner has over 20 years of experience. Her background has been family wellness, preventative care and emergency medicine. She started her degree at Oregon State University and finished at University of Portland. Lindsay has a passion for helping all her patients and you can tell the moment you meet her.

MEGAN LAJOY, DNP, FNP

Megan was born and raised in a small town in Eastern Oregon. She attended Linfield College and received a Bachelor of Science in Nursing. While practicing as a registered nurse for seven years Megan pursued her Doctorate of Nursing Practice Degree, and Family Nurse Practitioner certification from Gonzaga University. In her spare time Megan enjoys spending time with her family, chasing her dog around, traveling, and gardening.

GENEVA GREEN, PA-C

Geneva was born and raised in Portland, Oregon. She received a Bachelor of Science in Public Health from Oregon State University, as well as minors in Spanish and Chemistry. She then worked in Urgent Care and Dermatology prior to obtaining a Master's of Science in Physician Assistant Studies from Pacific University. Geneva is passionate about preventative health and is particularly interested in Dermatology and Endocrinology.

R. LUKE MOHLMAN, LCSW

Luke holds a Master of Social Work degree from Portland State University. As a licensed clinical social worker, he practices person-centered and solutions-focused therapy with an emphasis on mindfulness. He also employs Cognitive-Behavioral therapy techniques. Luke works to empower individuals to build on their strengths and he teaches skills that increase mental and emotional well-being.

HOW TO PAY FOR HEALTH CARE

Bridgeport Family Medicine payment policy:

Medical services are provided and billed by our clinic. Our providers participate in many of the major health insurance plans in the region. The following are the health plans which we currently accept. If your plan is not listed, we will be unable to file a claim on your behalf. However, we can provide you with a form that you can submit to your insurance company to instruct them to send reimbursements directly to you.  Please be aware that with any health plans, there may be deductibles and copayments for which you could be responsible.

QUESTIONS ABOUT YOUR BILL

What billing or insurance information will I receive?

You will receive a copy of your bill. After your insurance has paid, we'll provide you with a monthly statement of your account.

How long will it take to get things settled with the insurance company?

Generally, it takes from 45 to 60 days to obtain payment from an insurance carrier.

Why am I getting bills from physicians?

Each physician who provides professional services will send you a separate bill from his or her office. These physicians will include:

- Your own physician -

​

- Other physicians who conducted tests and procedures -

​

- Consulting physicians chosen by your primary physicians -

​

These physicians may include specialists in anesthesiology, cardiology, emergency medicine, nuclear medicine, pathology, radiology, or urodynamics. You will be responsible for paying these bills as well.

I received a notice that my insurance company has paid on my bill, but I can't understand how they calculated their payment amount. Do you know?

If we have received any such information from your insurance company, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you generally need to check with your insurance company.
